This follow-up is available* for Dr. Chow’s recent TBRHSC emergency department patients (within the last year) for ongoing or new health concerns if they do not have a primary care practitioner or cannot readily get an appointment with their family physician or nurse practitioner. This is for OHIP insured services and requires current OHIP coverage.

Only valid submissions will be registered and confirmation is provided from the Help Desk (helpdesk.chowtbrhsc@gmail.com) with access to a secure Patient Portal through the TELUS Collaborative Health Record at the Primary Care Transition Clinic.

The Patient Portal will allow you to send your OHIP identification if required and send or receive other private or confidential information securely. You can also book virtual or telephone Emergency Department follow-up with Dr. Chow.

*Other patients will need to access health care via their primary care practitioner, emergency department, walk in clinic or on-line virtual telehealth care options.
